If you’ve never worked with Julia Martyn before, pick a meeting date (in-person or virtual), and introduce yourself via a brief form for Julia Martyn to evaluate before confirming the appointment.
Appointments can be virtual online, or in-person at the office at 100 Conifer Hill Drive Suite #310, Danvers, MA.
Prior to scheduling your appointment, please review our FAQ section and read this page in its entirety. When you’re ready to schedule your intake evaluation, please click on “Book an Appointment” link at the top of our website, which will bring you to the booking section in our IntakeQ portal (the electronic health records system used by Wheeler Brook Psychiatric Care, PLLC). You will be asked to complete a brief form and will have the flexibility to choose between an in-office or online evaluation (please note that all prescriptions for controlled substances require an in-office evaluation). After receiving your request, we will proceed to either confirm the appointment, request additional information, or, if it appears that we may not be the ideal fit for your needs as a small private practice, we may need to decline the request. Communication regarding these matters will be sent via email through IntakeQ, so be sure to add this email address to your contacts or approved senders list to prevent our messages from being directed to your spam or junk folders.
Patient Portal Creation and New Patient Forms
Once your appointment is confirmed, we will email you a link to the intake packet, which contains several documents. These include practice policies, HIPAA forms, consent to treatment, and an intake questionnaire asking about your medical and psychiatric history and current symptoms. Please complete all forms thoroughly, so we can gain a comprehensive understanding of your medical and psychiatric history. Please make sure to include a detailed list of both current and past medications, including supplements/vitamins and over-the-counter medications.
All forms must be submitted within 72-hours of receiving confirmation for your appointment or your appointment will be cancelled. You will be able to reschedule once all forms have been submitted.
Insurance
Massachusetts & New Hampshire residents: We currently accept the following insurances: Aetna, United Healthcare, Optum, MGH PPO and HMO, Blue Cross Blue Shield, Harvard Pilgrim, Anthem Blue Cross.
We do not accept any MassHealth, Medicare, or Medicaid insurance plans at this time.
HelloAlma platform completes insurance verification and billing for all insurance plans we accept. Once you’re accepted into the practice we will direct you to their site so they can verify your insurance and handle billing for services.
If you do not have an accepted insurance plan, then we are Out of Network with your insurance. Please check with your insurance company for Out of Network coverage for mental health services. The best way to do this is to call the number on the back of your insurance card and request information about reimbursement before to making your appointment. Upon request you will be provided an itemized receipt of payment that can be submitted for reimbursement if you have out-of-network coverage.
Payment Information
We accept all major credit cards, cash, and personal checks. Cash and checks are only accepted during in-office appointments.
All patients are required to keep an updated credit card on file in the patient portal. Patients with an accepted insurance plan must also be enrolled in auto pay at HelloAlma for co-pays, deductibles, and late cancellation fees. New self-pay patients are expected to pay for the initial intake appointment by credit card at the time they book their visit. Patients are required to agree to our card on file policy prior to engaging in treatment with Wheeler Brook Psychiatric Care, PLLC.
Telehealth Visits
If we are having a Telehealth visit it’s important to plan ahead to ensure the visit goes smoothly. Please read through this section so that you’re prepared for your Telehealth visit.
-You may use a smart phone, tablet, or computer with a camera for our appointment. We are unable to provide audio-only telehealth appointments
-Please ensure that you have a reliable internet connection and that your device is fully charged.
-You will receive a link for our secure and HIPAA compliant video Telehealth platform (currently we use Zoom). It is expected that you are available at our scheduled appointment time and are prepared, focused and engaged in the session. We are not able to conduct telehealth appointment if you are driving – your appointment will need to be rescheduled and you will be responsible for paying the late cancellation fee.
